What Causes An Open Wound On The Breast Post A Reduction Surgery?

Hello, I had a breast reduction 5 weeks ago. Immediatly after removing surgery tape, my right breast had an open wound. Doctor said it was fine. Every week I saw my ps he would say it was ok. I noticed a foul smell, he said it was ok. Went to see other ps, had a ct scan done, blood work, debriment on wound, and will continue with treatment. My breast looks like a part of it is missing in the bottom. Like a chunk of meat missing. He also mentioned skin graft, but, it will be determined after open wound treatment , time, etc., What went wrong. Left breast is fine, only kind of red. I will ask for a CT Scan on that one too. Thanks
