My mother has Alzheimers related dementia. she was taking Risperidone 2x a day for several months, and the doctor just prescribed Seroquel 2x a day. He told me to take her off the Risperidone, but then after reviewing the discharge notes, it doesn t say anything about that. My question is: the last couple of days the small amount of awareness she had has dissipated and she is freely hallucinating. She also can t stand or walk this afternoon. (She was able to this morning). Is it possible that she suffered a TIA or is this a reaction to withrdrawl from the Risperidone?

Patients with advanced dementia often have hallucinations and agitation as part of the clinical picture. Risperidone is usually given precisely to control these phenomena (it does nothing for Alzheimer's disease itself, only alleviates these symptoms). So the symptoms which up to now were controlled by Risperidone have surfaced. It is not so much an withdrawal symptom, but no more drug effect. Seroquel should have a similar efficacy to Risperidone, with some differences in side effects. Perhaps a change in dosage could be considered or if even switch back to Risperidone if there wasn't a specific reason to interrupt it, talk to your doctor about that.
