Endocrinologist 's  Response
Good day,
Noted your concern. An endocrinologist is the best person. I presume your brother has acromegaly. This is a condition with growth hormone excess. Usually associated with a pituitary adenoma. If the pituitary mass is still there, then the options are

a) RE surgery ( preferred option)
b) Medicines
c) Radio therapy

These options should be discussed with an endocrinologist. Acromegaly is usually difficult to cure with a single operation. Treatement is possible with some injections and medicines. But usually not as effective as surgery .

Regards
Binu
