R Shoulder pain for about 18 months with marked increase the last 3 months (7-8/10) vs 3/10. Beside taken. Heat and ice applied. PT x1 month with no improvement. Able to complete most ROM however will have pain for hours afterward. Only thing I can remember doing is drawing myself onto my able and leaning across the bed and feeling like I pinched something in the pocket of my shoulder.
Noted your history of pain in shoulder joint for so long and the things tried with pinching pain while laying down in bed. The most probable causes I can think about are: Tenosynovitis. Ruptured shoulder capsule. Small fracture that is not healed. Arthritis of the joint and so on. I would advise you the following in such a situation: Clinical evaluation by an Orthopedic Surgeon. MRI and CT scan of the affected area. Relevant blood tests. All these will give you an exact diagnosis so that a proper management can be done. Do not over-strain the area as it can cause more harm.
