Suggest Treatment For Severe Cough And Breathing Difficulty

My bf has had severe cough for couple months now but now shortness of breath and difficulty breathing have become a problem too. He has very good appetite since he goes to the gym and works out but lately he barely eats says he feels bloated and when his sleeping he wakes up constantly due to shortness of breath and harsh cough attacks that keep both up at night. His heart rate isnt normal to my judgement and when he exhales he does it forced and harsh. I m not sure if his use of steroids may have something to do with it he only does cycles and never seemed to be a problem. He went to emergency and got diagnosed with pneumonia but didnt get any medication and this escalates more and more. What could be going on or the next steps that should be taken

Your brother is suffering from severe cough for a couple of days. His history is that he had got diagnosed with pneumonia but did not get it treated, didn't take any medicines. Presently, he is having difficulty in breathing, shortness of breath, heart rate not normal & is also bloating. Your brothers symptoms indicate that he is suffering from complication of pneumonia. He should have got treated for pneumonia in time.

He is suggested to get clinically examined by a GP, get sputum, swab and pus / mucus in the lungs tested to look for infection. He may require to get X-ray or CT done. Lung abscess, mucous/pus will be treated with antibiotics, if required it may need to be drained. His treatment will depend on the pathology detected and the severity of the symptoms.

The expected pathology that may be detected are empyema, pleural effusion, pleurisy, lung abscess, bacteremia and septic shock, etc. He may need to be treated with combinations of antibiotics like clindamycin and metronidazole. He can take PPIs, one stat when needed for relieve from bloating. He should be connected to a Physical therapist for lung exercises under observation.

He should take liquid / semi solid diet and stay adequately hydrated to prevent throat mucosa from drying. Cough lozenges and using a humidifier may be helpful. Since his heart rate is not normal, he is advised to get hospitalized and get thoroughly checked and treated. He needs urgent medical support.
