Suggest Treatment For Severe Abdominal Pain On The Lower Right Side

My daughter is 12 and has been suffering with what the gastro doctor calls IBS (only with constipation) for the past 2 years. She goes through bouts of severe abdominal pain which always goes undiagnosed at the hospitals. The doctors always send her to the ER for possible appendicitis. It never is. Well now she is suffering abdominal pain in her lower right side of her abdomen and in her back in same general area. It hurts to stand, cough, lift her arms, bend over and the pain is constant over the past two days. She feels it is getting worse. However, she is still eating and has no fever (two symptoms the doctors at the ER said she would have are fever and loss of appetite as well as pain so bad she can t sleep). She slept last night although she said she woke a few times but went back to sleep. My question is, should I take her to the ER again or wait to see if she gets a fever? I never get answers at the doctors or hospital as to diagnosis, so I only know what it has NOT been in the past, and she has never had appendicitis.

Pediatrician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

IBS (irritable bowel syndrome) is a functional bowel disorder. That means there is no underlying pathology detectable in bowel but bowel isnt functioning well. It may be due to abnormal sensitization to normal bowel function and may be it is recognized as painful by brain wrongly. The child needs change in diet in form of increasing fibers in diet i.e. roughage and increasing physical activity and outdoor sports. Avoid fast foods and unhealthy ready made food. increase salad and fruits in diet. Adequate water intake should be there. Please do ask your doctor for dietary counseling.

Hope I have answered your query. Let me know if I can assist you further.
