Suggest Treatment For Serious Memory Problems

I feel I have a serious memory issue. The other day my girlfriend ask me to turn the coffee pot on before I leave for work and I said ok. I walked out of the bedroom and it never entered my mind again until she text me later and said you did not turn on the coffee pot. This is but one example of many of the same instances over many years. I am almost 49 and feel like my memory has been bad for 15 or 20 years. What can it be???
