Suggest Treatment For Persistent Rash On The Ankle Area

I have had a rash for a year on my ankle area, I have had tests and biopsies which come back negative for parasite, fungus etc... the dermatologists and my gp have come to the conclusion of psoriasis or eczema. I also have a patch on my thumb and hand. I have had numerous nodules come up on my skin which the gp diagnosed as dermatofibromas, which were benign upon biopsy. If they are removed, they do grow back. Reading the reviews and questions, I am wondering if I also could have dermatomyositis. I had an xray last year and it did show interstitial pneumonia, I was treated with antibiotics, and the gp said that he would do another xray this year. I also have had some chest pain, which the cardiologist diagnosed as angina, after the catherization did not show any blockages. I know that without seeing pics of my rash it would be hard for you give me a clear diagnosis, but can you give me some ideas as to what to communicate with my doctor as I feel I am becoming my own advocate.

General Surgeon 's  Response
Hello,

Persistent skin rashes around ankle reappearing upon surgery as well most likely indicate auto immune disorder of some sort in majority of the cases. However, it is very unusual that even skin biopsy is not revealing any positive results.

Specific antibodies as anti J-1 or Mi 2 show up positive results in certain desmosome and hemidesmosome problems of the epidermal layers.

Treatment aspects would vary depending upon the relevant diagnosis as:
- Local steroid creams
- Immune therapy as imaquimod or others
- Boric acid or trichloracetic acid application
- Laser therapy or excision

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
