General & Family Physician 's  Response
welcome to Health care magic.
1.It looks like a suspicious mass lesion in the breast for which your doctor asked for a followup mammograms/ultrasound.
2.If you still feel the lump and pain - approach to the same doctor with a fresh ultrasound ( to find out the extension of the lesion)
3.then the needful action will be taken.
4.Yes, if required an excision/mastectomy -complete or partial depends on the involvement and type of the lesion.
Anything to ask ? do not hesitate. Thank you.
