My 97yr old Mother in Law is having acute lower abdominal pain. It comes at night in bed and lasts until she is awake and walking around. Sometimes it can come in the day when she eats something or ingests cold water. She has a history of bleeding Duodenal Ulcer that was cauterized a year ago and pain has started months after that. She takes a PPI and Sulfacrate but the pain persists. Her Family Practice Physician thinks that it s Intestinal Ischemia so she sleeps with O2 nasal canula. No help. There is CT scan evidence of Mesenteric Artery Occlusion at the proximal site but Opaquafication at the Distal portion from collaterals. She reports a pain level of 10 almost nightly and I m sorry to see her suffering so. Oxicodon plus 375 Acetimetaphin helps but she is in distress.
But we have to find out the cause of pain, and treat accordingly. For this, an abdominal examination and few investigations are must. Anyhow, this is unlikely from Duodenal ulcer, as the pain here will change with food intake, and it will be more towards epigastrium (upper abdomen). Well, mesenteric ischemia also can cause lower abdominal pain. Here also, pain will be more after food. Kindly discuss with your doctor.
