General Surgeon 's  Response
Hi.
Thanks for your query.
Noted your history of swollen/distended anal sphincter with leakage of stool and hence smelling like pooped. This has happened in past and steroids helps but your GP is not feeling so this time.
The probable causes are:
Rectal prolapse or prolapse of piles.
I would advise you the following:
Rush to ER as this can be a dangerous thing is long standing.
The Doctor, preferably a General Surgeon would help you to get this reduced manually, will have examination and give you the most probable diagnosis and hence helping you for the further plan of management to avoid the further complications.
I find this answer helpful
