Hello,

I believe your mother's doctor should consider improving the prophylactic power of the AMITRIPTYLINE by increasing her dosing to the following regimen which we commonly employ in the Headache Clinic practice and that is to actively UPTITRATE that medication by 10mg. weekly to be taken only at BEDTIME to a maximum of 50mg. nightly.

If she is only getting 5 mg. as you suggest then, this is an extremely small and likely ineffective dose for her headaches and this may be why she's not adequately responding.

I will also make the recommendation that if your mother complains of side effects using amitriptyline that she is switched over to NORTRIPTYLINE as this seems to be much more tolerable as we titrate upward in higher doses than amitriptyline.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Dariush Saghafi, Neurologist
