Suggest Treatment For AIDS

In July of 2006, my husband was diagnosed with AIDS. He was put on Atripla and since then has had a higher cd4 and undetectable viral load. Throughout this time, he has continued to use drugs, mainly crack, and has continually taken as many as 12 ibuprofens a day, a vitamin multipac, a vegetable pill multipac, 40mg nexium, 2 alka seltzer cold per day, and about 9-12 dr. peppers every day. In July of this year his kidney panel came back abnormal. His creatinine was 1.5 and eGFR was 53. His kidneys have been hurting for about 3 months and his urine has been foamy. In October, his test showed creatinine at 1.4 and eGFR at 57. This puts him in stage 3 CKD. I asked his doctor about this and he said it was wrong and that his eGFR was actually over 80. I don t know why the lab report would say differently. How quickly can this go to stage 5 if left untreated?

there are only 2 prominent reasons for switch in ART 1- Virologically failure( not applicable for your patient)
2-side-effects of Medicine
while on atripla one needs to be monitored regularly (6 monthly) kidney function to diagnose any tenofovir related side effect development and switch in art if needed.
egfr is calculated on basis of weight, age and creatinine of patient so won't be able to comment on exact egfr of your patient but in his case his other addictions may have had illeffects on renal function derangement and he needs urgent psychiatrist referral for addiction, and exact renal function diagnosis and if needed switch to some other art
