Suggest The Ideal Dosage For Lisinopril

HI, heart attack 2010 two stents left anterior & circumflex. My ejection fraction is 35% to 40%. I m on coreg 6.5mg twice a day & currently on lisinopril 5mg. It s been 7 yrs since my attack & my ejection fraction has not gotten any better. My new doctor wants to up my lisinopril to 40mg says this will help increase ejection F. I just had a nuclear stress went 8min on treadmill, doctor says I did good & no change from two yrs prior still 35% to 40%. Do you think increasing lisinopril will increase my ejection fraction. Also I heard that the Indian herb arjuna can strengthen the heart muscle do you think it would help. & can it be used along with the above meds. Thank You. Charles

General & Family Physician 's  Response
Hello,

Lisonipril is an antihypertensive drug belonging to angiotensin-converting enzyme inhibitors.

This, like the other ACEIs, is the antihypertensive of choice in patients who have suffered heart attacks, due to the fact that it has the property of improving the cardiac remodeling that occurs after the infarction.

So answering your question, rather than improving the fraction of ejection, what will do is improve the remodelling process, and this, obviously over time results in a fraction of ejection not deteriorated.

Regarding the Indian Herb Arjuana it would be better if you asked your treating doctor directly.

I could recommend it to you, because if you are right, there are studies that speak in favor of improving the functioning and cardiac contractility, but the ideal person to recommend it is your treating doctor.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Luis Alberto Urdaneta Arenas, General & Family Physician
