Hi. Thanks for your query. This is a rare problem and needs a proper evaluation as to the problems it can cause. Since the testicles were pulled down by your girlfriend, there is a chance that the cremaster muscle might have gone in to trauma causing it to relax and contract and be to the abdominal wall. This gives the appearance of them to be hanging lower that normal.
The pain can be due to the trauma of pulling , edema going to the places as you have mentioned.
The erection is normal means there is no ED on the part of the erection.
I would advise you the following with reference to the history you have provided: Get a color doppler examination of the scrotum and the abdomen to see what is the effect of pull . Anti-inflammatory medicines like Ibuprofen Scrotal support. and so on.
