General Surgeon 's  Response
Hi.
Thanks for your query for your husband and an elucidate history.

It is goof that the urine report has come negative.
Since there is a hydrocele- the extra fluid, the CT scan will tell us all detailed diagnosis which can be Orchitis with secondary hydrocele or Varicocele.

This will also tell us whether this is normal testes or not.

The treatment will depend upon the final diagnosis.
-Surgery if there is hydrocele
-Medicines for minimum 3 weeks if there is an inflammation of the testes.
-Surgery if varicocele is present
I find this answer helpful
