Occasionally I Get What Look Like Wells Filled With Fluid.

Occasionally I get what look like wells filled with fluid. They're tiny holes that go into the top few layers of my skin. They only ever occur on my big toe. They are extremely itchy. When I cut them open, a thin, clear fluid comes out. I have used Clotrimazole cream but it doesn't seem to be working. The big toenail associated with the fluid filled wells has also been very flaky and weak where strips of my toenail just peel off. Are these conditions related? What are they and how can I treat them? I've had them intermittently over probably two years and the itching it's just about unbearable. Please help!! Thank you!!
