Is Grand Mal Seizure In An Elderly Person Due To Carbamazepine?

I m an acute/critical care RN. My 94yr old mother had a grand mal seizure (2 minutes with 20min post ichtal phase) and has a history of right parietal stroke with sensory deficits left side, with left sided inattention and left visual field blindness. She has now had her second grand mal seizure. Her heart rate has been in the 40 s post event. She is on timolol 10mg oral, ramipril 10mg, amlodine 5mg, pantoloc and clopedirgel (hypertension, mild CHF post silent MI, angina, GERD). Should I have her MD or neurosurgeon consider adjusting dosages or alternate medications? We took her off carbemazapine (on for 3 weeks after 1st seizure) r/t severe drowsiness. Thanks Elaine

Hello,

The second attack of grand mal seizure is due to stoppage of anti-convulsant medication Carbamazapine without tapering it due to side effects of drowsiness. There are better and safe options available for the same as Valproic acid, Lemotrigine or others for her age group. Kindly consult her with an expert neurologist at earliest to prevent further episodes of seizure.
