Suggest Treatment For Extremely Bad Itching In And Around My Anus When There Is Dryness, Swelling And Slight Bleeding

In Feb 2018 I went to the doctor for extremely bad itching in/on my anus. Dry, swollen, bloody while wiping. Waking myself itching myself. This is the second time it has happened where I have been treated. I was treated for fungi with cream. It would come and go. In September, I was treated with thrush and more of the same cream, twice a day for 28 days. It has been two weeks and its not getting better. Could it be something worse?
