I'm Having Problem Remembering Certain Things More Often Than Usual

I'm having problem remembering certain things more often than usual and i keep getting things wrong even after checking it many times if i got it correct, once somebody else check it, it's wrong. for example, i have to type in "3.02" i check multiple times and even read it out loud and i see it or perceive it as correct but when i get it checked by somebody else, it's wrong. i cant seem to remember a lot keep a memory longer than i should, like if you ask me what i did yesterday or what i ate for breakfast, i wont remember it without something reminding me and sometimes, my vision blurs and i'd have to really focus to fix my eyesight again(i do wear glasses). i'm only 25 years old and i'm wondering if this is all normal. do i need to see a doctor? if so, what kin of doctor?
