I Have Pain Below My Right Shoulder Blade X 1-2

I have pain below my right shoulder blade x 1-2 months. In the beginning it was painful when moving to a standing position from lying down, or when turning in bed. I've self treated it with ice, heat, and various "rubs". In the last few days, it has become painful (sharp, grabbing pains like a spasm or cramp) with certain movements of arm or bending, and is more tender, like a bruise. I'm mostly concerned because I take a large amount of NSAIDs (for another condition). I'm 74, in good health, work out 4-5 times a week, eat mostly vegetables. I've thought this was muscular, but am concerned because it is not improving,,,actually getting worse. I guess my question is do I need an MD? I don't want to take muscle relaxers because of the side affects and am assuming that is what would be prescribed.
