I Have Hardly Been Sick A Day In My Life

I have hardly been sick a day in my life until I was diagnosed with a rare auto immune disease 18 months ago, dermatomyositis. Been through all kinds of stuff, I also have a finger on each hand with gangrene and amputation and sympathetic hand surgery is coming up in a couple weeks. I have Interstitial Lung Disease which is brought on by the Dermatomyositis.I have had swollen ankles before due to medications etc. About 3 weeks ago I got bad swollen ankles and feet (totally different than in the past) a red splotch or two and it hurts very bad to touch the skin at the top of feet, ankles and shin bone area, The swelling goes down at night and comes back immediately when I wake up. My rheumatologist thought it might be cellulitis and I took an antibiotic for 10 days and it did not help at all. I am not sure it is cellulitis and I need to know what to do now, what specialty of doctor to see. What else it could be besides cellulitis. I am taking a blood thiiner to try and thin blood to get to hands and it seems like it has helped hands to stop adding more fingers with gangrene so not sure it could be blood clots. I would love some advise, what to do, where to go, what it could possibly be that is causing it. YYYY@YYYY
