I Have A Problem With My Left Hand, Especially The

I have a problem with my left hand, especially the thumb; it is drawn, sometimes has swelling into the palm of my hand. this has been an on going theing since early January; later there is pain in the upper arm, almost to the shoulder; It does have some pain when attempting to put clothing over my head, such as a blouse. I was referred by my internist to an orthopedic surgeon who did mention the possibility of surgery or therapy which I definitely would prefer therapy over surgery- also I really thought he meant surgery on the hand, but later realized he was thinking shoulder as he believes the rotator cuff is involved. Considering my age (82) I do not wish to have surgery if there is any other way. Please advise your thoughts.
