I Had Pituitary Surgery In 2009. I've Been Under Stress

I had pituitary surgery in 2009. I've been under stress lately and started taking 20mg of my hydrocortisone but I'm having problems with swollen ankles, low blood pressure, headaches, irregular heartbeat, serious fatigue to the point that I have to sleep or I'll pass out, nausea and a terrible taste in my mouth. Should I go to the ER? Is this something other than adrenal insufficiency?
