I Am HLA-B27 And Have Been On C2 Pain Rx

I am HLA-B27 And have been on C2 pain Rx since 2004. Currently
On pain Rx, SubQ Methotrexate, and Remicade infusions.

A MRI done today was unremarkable. No bamboo spine appearance, loss of vertebral height or fusion. According to pain management physician without lumbar damage I don't need pain medications , lumbar nerve ablations, or lumbar shots just based on the MRI results.

Isn't the purpose or Remicade and Methotrexate to prevent disease progression? If progression is slowed and isn't visible on an MRI is that supposed to make me Pain free and not need narcotics. Isn't the free floating TNF still causing inflammation and thus pain?? Can there be inflammation directly caused by HLA-B27 that causes pain but hasn't caused spinal damage yet.

Doesn't the inflammatory process still cause pain in the absence of MRI changes and thus qualify for pain management.
