How To Treat Recurrent Fever?

My 4 year old son is experiencing since 2 years a monthly reccurent fever that reaches 40 degrees and lasts for 3 days with no other symptoms. After the consultation of a pediatrician, an infectious disease specialist, and a hematologist and extensive lab tests ranging from CBC (MCV= 65 and microcytosis) Immunoglobulins (normal) CRP (75 - 160) FMF (no mutations of the tested genes), all doctors agreed to put him on colshicine and promised he will be better. However after 4 months on colchisine he still experience the same reccurent fever with no improvement. I kindly ask you to guide me so I can find out my son s problem and a solution. I think he must have been screened for all known infections esp malaria. I think he need consultation with rheumatologist.
