How To Switch From Acitrom To Warf?

My dad (82) suffered a stroke in Jan. He was on Warf 5 thereafter which was reduced to Warf 2 and then stopped after 2 months on doctor s prescription. However he soon suffered a second stroke in April. He has been prescribed Acitrom 2 now. But Acitrom has a lot of side effects. My question is whether we can switch to Warf from Acitrom and if yes, then how?

Hi,

To answer your question directly, yes you can switch between the two. However, it would be prudent to inform the treating doctor about your decision.

You can start with the same dose of the new medicine. Then monitor PT INR which should be maintained in the range suggested by your treating doctor.
