Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here are my thoughts.
The puffiness under the lower eyelashes on only one eye can be due to the following reasons: Vascular anomaly like arterio-venous fistula or hemangioma. Accumulation of excessive fat Some sort of tumor. This can be diagnosed by the following: Consult an eye Surgeon - Ophthalmologist. MRI and high resolution ultrasonography. These will give an exact diagnosis and will explore the possibility whether surgery will help you or not. IF surgery helps to get a better result so that both sides look same, go for it.
Hope I have answered your query. Let me know if I can assist you further.
