masses full thickness tear of the whole subscapularis tendon with associated atrophy of the musel belly suggestive of a chronic tear ! complete full thickness tear of the whole subscapularis tendon with tendon retraction and a complete tear of the long head biceps tendon with tendon retraction! and large joint effusion

You are having complete muscle tear and so joint effusion as well.

CT scan report should be done to look for grade of muscle tendon injury.

Intralesional steroid can be given to relieve inflammation

If age and clinical condition permit than surgical treatment is needed
