How Can Brachial Plexopathy Be Treated?

I have bilateral brachial plexopathy resulting from full course of radiation therapy 23 years ago. Pain has increased substantially over past 2 years. Until 2 weeks ago I was prescribed nasal butorphanol. I recently started with buprenorphine. The original dose of 2 mg was overwhelming, resulting in excessive dizzyness and nausea. I cut the pill (sublingual, dissolving) in half (with pain doc, approval). Same symptoms, I would like to further reduce the dose, but because of the shape of the tablet, getting 4 equal doses is unlikely. Is there a form of this drug available in a lower lose (like 500 mcg)? I currently take this medication at 6 PM, and when I awake in the morning ( 9AM) I am dizzy for a couple of hours and occasionally experience nocturnal emesis. Several days of this treatment, I have gone to sleep at 10 PM, awoke at 10 AM, napped from 12 to 5, then back to sleep at 9 PM. Ideally, I would like a dose of medicine that I could take in the morning and experience no side effect and be able to play golf, I am a retired (16 yrs), 71 year old endocrinologist

-Avoid burns or cuts, as you may not feel them if you're experiencing numbness
-surgery may be needed if the symptoms does not recover
