Hello, I Have An Embarrassing Question. I'm Currently Experiencing Pain

Hello, I have an embarrassing question. I'm currently experiencing pain in my rectum that fluctuates throughout the day (4 out of 10 to 8 out of 10) depending on my activity and position. I saw my PCP about 2 months ago about a mass in this area. He believes it may be a thrombosed hemorrhoid so he referred me to an imaging center and a gastroenterologist. I'm currently a student and can't afford health insurance so I missed my appointments because i couldn't afford them. Last night I couldn't sleep from the intense pain. I was Up until 6am when I knocked out from exhaustion. I have trouble going to the restroom. Sometimes the toilet is filled with blood. It hurts to stand, walk, sit, lay down etc. I called one of the Emergency rooms because I want this to stop and a nurse on the phone just told me to take Ibuprophen...
Ive taken ibuprophen, tylenol, Perp-H, Warm baths, and nothing helps anymore. The mass is about the size of a nickle, dark purple, protruding form the anus, and cannot be pushed in. What do I do?!?
Can an ER doctor remove it and I workout a payment plan with the hospital?
