Dear Doctor, I Am A 58 Year Old Male 171

Dear doctor,
I am a 58 year old male 171 cm tall and weigh 68 kgs ,of medium build and generally in good health except for high sugar levels for which I am under the following medication for the last 5 years:
1.Glycomet GP 3/850.............................1-0-0
2.Istamet 50/500....................................0-1-0
With the above,the blood sugar levels were moderately under control with fasting level at 100-125 and PP lvl at 150-170.
Since last one month,the levels have spiked up to- fasting 250 and PP 350 and are not coming down.I am a vegetarian and do not consume sweets or refined sugar or any soft drinks/pastries/icecream etc but I do consume fruits like Banana,Papaya,Citrus fruits and Apple.
I have missed some 45 min walks in this period. I am not under any medication other than the above and the comprehensive blood test report done a couple of months ago and the report is attached.
Can you please identify the probable cause and suggest suitable medication/change and treatment and dietary adjustments to be made if any.
