Child Having Bloody String Coming Out While Urinating. What Could It Be?

my son is 13. He went to the bathroom to pee and he noticed a red like stringy thing come out while he was urinating. it looked like a bloody string he said. he said it was the first time it happened. what could it be and should I make an appointment.
