General Surgeon 's  Response
Hi.
This is but a rare question.
Loose scrotum with progressing age. Due to that the balls hand low, feels sometimes too tight.
Since you are asking about a desired underwear for a big scrotum, I have a feeling that you have hydrocele.
Hence I will advise you the following:
Ultrasonography and color doppler examination.
Consultation of a General surgeon to see whether this is hydrocele, varicocele, or just an elongated scrotal sac.
Surgery may be needed if advise by your Surgeon and get it done.
I find this answer helpful
