Hi welcome to health care magic....

You are having history of mastectomy...
So there might be breast cancer present in past....

Just now enlarged lymph node and new lump suggestive toward metastatic spread of tumour.....
Residual malignant cell might proliferate and leads metastatic spread...

So consultation needed with opinion of oncosurgeon and accordingly treatment given...

To rule out nature of lesion FNAC done or biopsy also useful for evaluate your case....

Take care..

Advise : oncosurgeon consultation for examination
