83 Yr Old Female Relative, 130#, Is Basically Healthy With

83 yr old female relative, 130#, is basically healthy with hypertension main problem and slight dementia. She is taking amlodipine 5mg, Losartan Pot 25mg, and Triamterene/HCTZ each 1 time/day. Occasionally her BP does get elevated, esp systolic to high 100s, but also can drop low causing dizziness. I'm concerned maybe too many BP meds. Just wanted an opinion on this. She is not a good eater not drinking of water. Recently hospitalized with dehydration and low blood sugar. Had basically passed out and trembling for short period.
Thanks
