I had a lower back injury in October 2015 , I had p.t for a few months, but off and on I still feel the effect off it, this year I started to feel the pain radiating into the him and a few months ago into my groin. It goes and come. I was diagnosed with ulnar tear.

Now every thing comes clear.
This situation comes from past injury.
An X-ray or MRI report of the past injury can clarify the diagnosis.
Now an inflammation may have started and or worsening of a herniation of the disc causing a pinched spinal nerve.
You can try to have rest an use ice compresses to reduce inflammation.
Physical therapy under supervision may be beneficial too.
Use anti-inflammatory drugs such as Advil or Naproxen to reduce inflammation and pain for the time being.
Maybe a new x-ray or MRI of the back is needed.
Ulnar tear is related to only arm pain.
