Brief Answer:
Certain characteristics can usually distinguish between ortho and gastro

Detailed Answer:
Hello, I'm Dr. Branch, thanks for using 'Ask a Doctor'. Usually there are a few characteristics that can help tell whether a problem is coming from the musculoskeletal system, or the gastrointestinal system. For example, if the pain is worsened with movement or use of the involved muscles, it is likely an ortho problem, while if movement does not make it worse, it is much less likely to be ortho. Also, if you are having any change in your bowel movements, such as diarrhea, or constipation, that makes gastro more likely, while if you are having normal bowel movements, and the pain does not seem to be associated with eating at all, this would make gastro less likely.

If you can give more details about the pain, I may be able to help determine what the cause of the pain might be. Ultimately, an examination may be necessary to confirm clearly what the cause is.

I hope that helps, please let me know if you have any other questions about that, and I would be glad to discuss it with you further.
