Question: my sister is 55 years. heavy smoker. she is hypertensive and lately at 170/110 mmHg most of the time though taking Exforge HCT 10/170/25. her thyroid is enlarged and affecting her breathing . today she suffered fatigue and high blood pressure after an exceptional effort she exerted with some chest pain. she went to the ER and attached are her ECG and CK analyses. is her coronary condition acute or chronic? and what is your assessment

I passed carefully through your sister's medical history and uploaded tests and would like to explain that her recent clinical symptomatology is caused by uncontrolled thyroid function.

Thyroid gland dysfunction may lead to unbalanced thyroid hormones production with their underlying consequences like persistent hypertension, tachycardia, cardiac arrhythmia (like frequent extrasystoles, atrial fibrillation, etc.)

If left untreated for a long period of time, thyroid dysfunction may lead to cardiomyopathy with permanent structural cardiac disorders.

Normal troponin level rules out an acute coronary syndrome (CK-MB is borderline too).

So, to conclude, it seems that your sister's condition is likely to be chronic; anyway the exacerbation of hypertension and cardiac arrhythmia are a call for immediate medical intervention through a prompt therapy optimization.

Coming to this point, a review of her thyroid function is required through blood thyroid hormones level measurements.

In addition, cardiac function should be checked through cardiac ultrasound test.

A beta-blocker (like Atenolol, Metoprolol, or Carvedilol, etc.) should be started if no contraindications exist; coupled with specific thyroid therapy.

Anti-hypertensive therapy should be optimized by modifying the current daily doses and/or starting additional drug(s).
