Question: hi, The last couple of days ive had some severe pain. its been in the lower abdomen area, like severe period cramping but much worse. its been dulled a little (not much) by panadol and advil, but it feels like my lower abdomen is being ripped appart. ive had some spotting a week before.. and some constipation. (since then the constipation is all gone) but its very hard to walk, i had some nausea too. just wondering what this could be and what tests or things u reccomend? thank you

Noted the history well and understood the concerns.
Such a severe pain in lower abdomen that it is making walking difficult, feeling like lower abdomen is being ripped apart, no constipation now, and some spotting a week before may be due to one of the following reasons:
Twisted ovarian cyst.
Volvulus
PID that is pelvic inflammatory disease.
Any vascular problem with lower abdominal contents like intestine and so on.

I would advise you the following in such a situation:
Rush to ER for clinical evaluation, examination and investigations:
X-ray of abdomen in standing position and ultrasonography of abdomen can be indicative of what the problem may be.
This added on by the proper clinical examination can give the need for further investigations like CT scan of abdomen and as needed.
This will give the plan of proper management as per the findings.
