What Does My Lab Test Report Indicate?

Question: Hello,

I recently had an Activated Protein C Resistance lab test to see if I had Factor V Laiden so I could get on birth control pills. This does run in my family. The lab results said that it was abnormal and low, being 1.90, where 2.3 was normal. My gynaecologist said it was fine and they just flagged that so she would read it. But, I can't help but think that it said it was low and abnormal. Is there reason for concern?

Thanks for your query.

Though you need to follow the reference range provided by your lab, usually 1.5 - 2.0 denotes borderline. As per your lab also, 1.9 against a normal of 2.3 appears borderline. However, you said that it rums in your family. Hence in this setting you should get Factor V Leiden assay by PCR technique.

If there was no family history then it could have been just followed up. But with family history it is better to investigate fully. So probably you need a hematology consult.

There are other causes too

This is only a screening test. There are causes other than factor v leiden, like protein c or protein s deficiency, antiphospholipid antibodies, increased estrogen levels, cancer, etc.

Protein C or S deficiency needs genetic testing. Other causes are pretty obvious from history and clinical examination, or specific tests.

So this test only screens for Factor V Leiden? But the other genetic testing would tell more. I am asking because I have had ENT problems for two years, I just wonder could my 1.9 borderline point to any other health problem. Basically,

Does 1.9 warrant any other testing, b/c it does state that it's abnormal on my blood work. I have had reflux, and ENT issues for 2 years, and I am going on birth control to try and level out my hormones, thinking it might help with my current situation. I have been lost trying to figure out why my throat has hurt for 2 years straight, so if this 1.9 APRCR result can point to anything else that may be causing problems I'd like to know which way to go!

Yes basically for factor v Leiden

As I have mentioned before, you need PCR test for factor v Leiden. Its only effect is the predisposition to blood clots. It has no effect on ENT problems. Since you have family history, the docs want to rule out this before giving birth control pills, as these can increase risk of clots if factor v Leiden is there.

I feel that you need PCR test to rule out factor v Leiden conclusively, before going on birth control pills.
