Question:
MRI REPORT REVEALS THAT THERE IS HERNIATION OF THE RIGHT FRONTAL LOBE INCLUDING BASIFRONTAL AREA THROUGH THE DEFECT IN THE FORNTAL BONE IN RIGHT PARAMIDLINE REGION. HYPERINTENSE AREAS ARE SEEN AROUND THE HERNIATED BRAIN POSSIBILY REPRESENTING CSF & MENINGES. THE RIGHT SIDED CRIBRIFORM PLATE & LATERAL LAMELLA OF THE ETHMOID ARE DEFORMED. CRISTA GALLI APPEAR NORMAL. NO ANY DEMONSTRABLE EXTENSION TO THE NASAL CAVITY IS SEEN. SIR, ONLY 10 MONTH CHILD, WHAT IS PROPER TREATMENT AND PRECAUTION SHOULD BE TAKEN.

2. The frontal lobe of brain is herniating through the defect and is seen protruding through the area and is inferiorly placed.

3. There is also deformity in the bones of the area above the nose region which is called cribriform plate and crista galli.

4. A neurosurgeon consultation is suggested keeping in mind a reconstructive surgery to prevent consequences due to brain herniation.

5. Since you have mentioned that you have shown to a doctor and they have suggested surgery, I want to know complete details so that I can discuss it in detail with you. Medicine cannot treat this condition.

1. The problem which your child is having is a rare one and about one baby in 40000 might have such a condition.

2. There are many variations of the problem depending on the abnormal development of the structures in the anterior base of skull.

3. As you know there is a portion of the bone above the right eye which has not developed completely and therefore the brain is protruding from within the base of skull.

4. I have seen few cases similar to the one like your child and have worked with neurosurgeons and plastic surgeons involved in repair surgery. Nearly all patients who get operated for this condition lead a normal and healthy life.

5. Surgery is the best way to treat the condition and the method of surgery is decided by the neurosurgeon after seeing all the MRI images. In some cases the trans nasal approach is possible while in most cases the trans cranial approach is used.

6. The surgeries are planned and highly successful and almost every patient has a good result following surgery.

Sir, is medication treatment possible without surgery because my child is only of 10 months. I am fear, for developing other complication after surgery as reported in another cases. Treament of my child, if possible whithout surgery, please adviced suitably and also Docter's name.

1. I understand that your child is very small and 10 months old. It is true that few patients getting this surgery might develop complications which might require more treatment measures.

2. This condition is due to non development of part of the frontal bone which is present above the eyes. Giving medicine cannot help to repair the defect because it is a structural problem.

3. I will suggest that you take your child to a tertiary neurology and neurosurgery research centre like NIMHANS, XXXXXXX and have a detailed discussion with expert neurosurgeons. It is possible to plan the surgery after some time when the baby is little older. They will also clinically examine your child and then suggest the right measures for surgery and any other latest treatment options.
