Question: My wife is complaining about being cold and shivering with two comforters on her, and her knee hurts because of a total knee replacement earlier this year and its not healed yet

Could be wound infection.

Detailed Answer:
Hi I am Dr Gopal Goel Orthopedic Surgeon.

Your wife had knee replacement one year back and has cold and shivering .Also the wound has not healed yet.

In this case my first suspicion would be wound infection with fever. I will get X rays , Blood counts , blood and wound culture and start broad spectrum antibiotics. Rest will depend on the findings in reports.

In my opinion , you get the above tests , consult your surgeon and let us know also.
