Brief Answer:
Physical examination and Investigations .

Detailed Answer:
Hi.
Thanks for your query, Sir.

Very small query- pain on the lower right side. From the other history I could understand that the Upper GI endoscopy and the colonoscopy were performed.

May I please know a detailed history:
Since when have you got this pain?
Any other symptoms like nausea, vomiting, distension of the abdomen or fever?
When was the hernia repair done?
Any problem related to to pass urine and / or stool ?

The causes of the pain in the lower right abdomen can be :
-Entero-colitis.
-Appendicitis.
-Stone in the right ureter.

I would suggest you the following:
Ultrasonography and CT scan of the abdomen.
Investigations of the blood, urine and stool.

A course of an antibiotic and metronidazole., activated charcoal tablets, Probiotic , painkillers if needed. See the results within 5 days.

Once the diagnosis of the cause is found, the treatment will easy and curative. It may be medical if no surgical cause is found.

I hope this answer helps you to get a proper guidelines for diagnosis and treatment. Please feel to ask for more if you need to.
