Brief Answer:
STOP Otrivin immediately..have faith..you can do it..

Detailed Answer:
Hello XXXX, Welcome back to HCM,

I've gone through your query and understand your concern,

Your symptoms and the fact that you can't do without otrivin, resembling a typical medical condition i.e. rhinitis medicamentosa (an extensive form of Rebound Nasal Congestion due to excessive use of Nasal decongestants like otrivin)..

The first and foremost thing to overcome this is, STOP Otrivin immediately.. it's tough and Cold- turkey expected, but you'll recover (time duration varies from person to person, depending on how long has been abusing and how severely affected), it's the only way to get over it safely.. have faith..you can do it..

Second step is, buy Saline nasal spray and nasal drops, and start putting 2 drops in each nostril initially every 4th hrly, then gradually titrate the dosage to 6th hrly, 8th hrly and so on (apply as long as you feel like, till this phase is over)..

You can also start taking steam inhalation three times daily for next 7-10 days and observe how you stay..

Meanwhile it's extremely important that you get it checked once with the ENT specialist as he/she'll inspect the nose under direct vision, which is extremely important as it'll give a clear picture regarding the severity and future course of management (if extremely severe, then surgery is the choice, however before that a trial course of Steroid nasal spray and/ oral steroid therapy is advisable)..

I can also see that there's a significant amount of smoking involved (20 Cigarettes a day is quite a lot and significant enough to aggravate all these symptoms, which I believe you must be knowing already), time to stop smoking at the earliest..

Let me know if there's any queries..

Take Care
Kind Regards
