Question: Hi Doctor.
I'm 45-Male. I'm IT Project Manager by profession and from Nepal (the South Asia). I'm having mild and constant pain in my belly button area and lower right of belly. This is from last 6-7 years which flairs up timely and remain relaxed some times. I'd been diagnosed 4 years back with Gastroenterologist. I went across, Stool, Blood, Ultrasound and even CT Scan of Abdomen. Every thing was fine and doctor just suggested me for the caring of life style.

Recently 6 months back again I went for diagnosis as its just let me feel insecure because of continuous flair up. This time again I visited Gastroenterologiest and went through the same procedure. We did all the stool, blood, ultrasound, CT Scan again and more onto that Colonoscopy. Doctor couldn't find any abnormality. He left me with advise that, if i wish he can plan the removal of Appendicitis, which have resulted positive result in the many similar cases like of mine.

I'm quite confused and in worry about the pain which is real mild but keep me distracted. I can't ignore but I can work Normal also. Sometime When it seems bit worsen I take Cifran (CIPROFLOXACIN Tab) and/or Bifilac () Capsules, which brings things back to relax. (This was prescribed by the consulting Doctors). I wonder If nothing can be diagnosed then why I feel pain and how Antibiotic like Cifran helps it to reduce! I also worry that consumption of Cifran and/or Bifilac (time to time can left adverse effect on my health)

Please help.

Brief Answer:
Rifaximin and bifilac

Detailed Answer:
Hello i have gone through the description you have provided. Your symptoms could be because of irritable bowel syndrome. In this condition there is visceral hypersensitivity which leads to excessive perception of pain. Bifilac wont cause any adverse side effects.

I would suggest antibiotic like rifaxamin along with antispasmodic like mebeverine and Bifilac. it shoul help to control the symptoms. You will need a prescription for rifaximin.

In case your symptoms persist after these medication you will need pain modulation agents like tricyclics.

Hope this helps

Thanks Doctor for your response.

To be just clear ...

1. I wonder, why my consultant thinks removal of Appendicitis may give relieve? Seeing the symptoms do you also agree removal of Appendicitis? Is it possible that my Appendicitis had been ruptured and healed by itself in past, which might be troubling time to time?

2. About your recommendation, what is the ideal dose of Rifaximin? Having its dose with bifilac is a temporary cure or permanent? Do i be ready to expect such flair up again in future after the recommended dose?

3. Is there any preventive method that can help me to prevent the re-occurrence. What would be the impact of bearing this decease on my health for longer? can it evolve any other abdominal issues in future?

Thanks for your time and words,

Regards,

Brief Answer:
Doesn't sound like appendicitis

Detailed Answer:
hello
the CT scan would have showed evidence of appendicitis either past perforation ar present swelling. So based on your description it doesnt sound like appendicitis.
I use rifaximin in a dose of 550 mg Twice a day. but is is a prescription drug and you should visit your doctor to get a prescription.
it is not a permanent solution. but it can give longstanding relief. your symptoms might recurring and if they do then your medications can be adjusted depending on the symptoms.
avoid alcohol tobacco deep fried foods and carbonated drinks. regular exercise( aerobic) like walking will help.
this problem should not affect your general health status
hope this helps

Dear Doctor,

Thanks for your opinion. I'm just back at home from work and got access to my reports which are from last 6 months. I've not undergone again to any consultant recently.

I thought uploading my all last report may help you to be more precise over on your opinion. Also who knows you may see something which my consultant could have missed.

Kindly please go through the reports as needed and have your opinion please. Based on your opinion I'll see my local consultant again.
