Question: My 95 year old Mom is constantly holding her back at the kidney area when she walks. She can't get comfortable sitting down and uses a lot of padding on the seat. She frequently feels the urge to urinate and runs to the bathroom with nothing coming out. She says she is not in pain, but I am concerned.

Hello,

her symptoms suggest urinary tract pathology. Urinary frequency is usually caused by lower urinary tract infections (lower UTI). Lower UTI do not usually cause kidney pain. When there is obstruction of the urinary tract (tumor, stone, etc) there could be both pain and frequency. Finally an upper UTI may present with pain and frequency in old individuals. In younger ones high fever is the most prominent symptom.

It would help to get her to do a urinalysis and urinary tract ultrasound scan. The urinalysis may detect blood, white blood cells, etc and provide clues to the diagnosis. The ultrasound scan may detect obstruction, stones, tumors and help to choose the appropriate next step in management.

If she has a UTI then antibiotic treatment would be required. If there's obstruction (due to any cause) then more detailed assessment would be required (CT urography).
