Question: I am 70 years old and over the past 4 months I have not had and ejection during intercourse. What do you suggest I do?

Dear Dr. Heijebu

This information is a reply to the above-mentioned brief answer is as follows:

My doctors have prescribed Cialis but my insurance company has not approved it because the doctors didn't provide basis for the prescription.

For your information over the past 4 years I have had the replacement of my ascending aorta and descending aorta at the Clinic.

Questions:

I. have controlled Hypertension
2.List of medication are as follows:
amlodipine 5mg
Aspirin 81mg
atorvastatin 40mg
carvedilol 6.25 mg twice a day
Valium 5mg nightly
vitamin D one capsule weekly
gabapentin 600mg twice a day
B complex
psyllium 3 times a week
sertraline 100mg daily

3. No tests taken
4. 15lb overweight

During the statement of symptoms, please note I had an erection, but no ejection.

May be drug induced side effects. Further evaluation is required

Hello,

As per your symptoms you have difficulty in ejaculation.

Ejaculatiry disorder in your case could possibly be related to side effects of the drugs which you are taking.

Drugs particularly Sertraline and antihypertensive agents can contribute to this side effect.

Reduction of drug dosage of Sertraline or Drug holiday on a weekend will help to improve your symptoms.

Likewise overweight might be additional contribution. Hence weight reduction is advisable.

In all cases the obstruction component in the Genitourinary tract should be ruled out by appropriate like Ultrasound abdomen/pelvis and Scrotal Ultrasound Doppler study. Further specific management if any is largely based on test reports.

Please check with your physician if he shares my view and if can prescribe these tests to you.
