Question: UROLOGY PROBLEM

1. I am a 44 year old male. My weight is 82 kg , height 6 ft 2 inch with athletic body. I like to do physical activities to keep fit and take balanced diet . I am having mild pain for last 3 weeks in my left testicle. Sometimes it subsides for short intervals. However, After some physical activity it tends to go up slightly. I have not taken any painkillers.

2. There is no urologist within 2 hours of my place of residence. I examined the testicle on my own after seeing from Internet. There is no Visible swelling, lump or redness on my testicle to the best of my understanding. However the left testicle appears bit tender to touch and on one specific point the slight more pain is there when touched. It also appears bigger than the right one and also is above the level of right one. The scrotum a lot of times feels tight and heavy. There is no urology related problem in my parents. However there is a problem of having too many non cancerous growth of lumps at lot of places in the body which increase with age, especially on arms. I have few on them too in my body near abdomen, arm and side back. Now I feel such small lump in my scrotum too near groin.

3. I got two ultra sound done for similar thing in 2017 and 2018 while consulting a physician. They couldn’t diagnose anything. The pain subsided on its own then. However, now this has recurred.

4. I also have a problem of requiring to urinate soon after I drink water ( I tend to drink less water because of less thirst while I want to drink more). Also my urine stream has gone relatively weaker in last 3 years. Also I feel that my bladder does not hold that much urine as others can since as long as I can remember. If I drink a glass of water at night, I get up at least twice to urinate. There doesn’t seem any other urine related discomfort.

5. I have tried to explain all I could as regards my problem. Can you suggest some relief from the pain. I can avail urologist facility only when consultant urologist start visiting Nilgiris (Ooty), Tamilnadu area after COVID.

5. All other vitals are normal except for occasional high pulse around 90 to 100.

Thanks and regards

Brief Answer:
Pain in Left Testicle could be due to Orchitis

Detailed Answer:
Hello
Thanks for query .

Based on the facts that you have posted pain in left Testicle that you have could be mostly due to mild infection of Testicle (Orchitis) which is secondary to UTI caused by Dehydration however diagnosis can be confirmed only on clinical examination and relevant tests mentioned below..

If you have access to General Surgeon or qualified General Physician consult him for clinical examination and get following basic tests done to establish diagnosis .
1) Urine routine and Urine culture
2) Ultrasound Scanning of abdomen an Scrotum .

In the mean time stat taking antibiotics like Ciprofloxacin and anti inflammatory drug Diclofenac twice daily for a week along with urine alkalizer Citralka thrice daily. .

Ensure to drink more water to keep yourself well hydrated .

The multiple lumps that you have all over he body are most likely Neurofibromas which are hereditary and benign in nature .

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.
