Question: Hello Doctor,

We are planning to have protected sex. Could you please suggest the best methods.
I would like to give some medical background about my wife. I have been married for 8 months. My wife had conceived spontaneously after 3 months of marriage . Unfortunately , we had to abort the baby at 14-15week of pregnancy. My Gynecologist found some abnormalities the fetal so we had to terminate the pregnancy. Doctor suggested not to plan pregnancy for 3 to 4 months. We are okay for that. We would like to know the best options to have intercourse to avoid pregnancy. I here by attaching all reports of my wife last pregnancy . Please check it and let me know if you have any further questions.

Thank you Dr, really appreciate your prompt reply. I have few questions regarding my next steps.

Upon my OBGYN we have sent aborted fetal to for further examination. We have sent aborted fetus to NIMS , XXXXXXX They sent it for genetic analysis to find out the causes. We are not sure what was the problem but hope to see the results after 6 months. The tests they are going to perform are Fetal autopsy , LARGE SP >3 BLOCKS , DIGITAL X RAY SINGLE VIEW . They told me that this whole process would take around 6 to 7 months. They asked us to contact around 27 Feb 2017.

I would like to know if you can suggest best genetic counsellor in my locality.

I reside in XXXXXXX Telangana state.

Do you think we (my wife , I ) has to undergo any medication before we plan for next pregnancy . I would like to understand your view and suggest me to proceed further.

Hello Madam,

My marriage was a consanguineous marriages . My wife is my first cousin.

Basically my wife is my mother's brother's daughter. We got married on 10th Feb 2016. My wife conceived on 17th Jun 2016. She had last menses on 13th Jun 2016. We went for urinal test as she missed her period. Urinal test was positive and doctor confirmed that she conceived. Then we went for Ultrasonogrphy early gestation test followed by UGC NT Scan. Both of these scans were done in July and August 2016. I have attached both the scan results. The doctor said everything is normal need not worry as of now.

As part of 3rd consultation we contacted doctor Dr. V XXXXXXX MD OBG. She found that there is some abnormalities in fetus which they call it as "Primi C 4mA" ( I have attached the prescription screenshot)
. Then she asked us to go for couple of ultrasound scans and Obstetric scan. She said that the urinal bladder of the fetus was over distended measuring 16mm. After that we had Ultrasound Pregnancy - Antenatal Scan on 22 Sep 2016. Both results were of the same. Then doctor suggested not to continue as there is heavy risk involved in it.

I have contacted Health Care Magic and uploaded all the reports for suggestions. One of the OBG specialist called Dr. XXXXXXX XXXXXXX . After looking at all the tests , scan reports she said that this is called as Fetal megacystis with echogenicity of kidneys. She said the decision is our's whether to continue or abort it. After consultation with my family and friends we decided to abort it.

We aborted it on 25th Sep 2016 at 11.05 pm. Upon Doctors order we sent aborted fetus to NIMS XXXXXXX for further testing. I here by attaching the doctors order .

Please look into all the reports and tell me the next steps to have a healthy baby.

I have reviewed the reports. They are suggestive of posterior urethral valve in the fetus which is associated with chromosomal abnormalities in 6 % cases.
There is no way to prevent it and chances are higher with consanguineous marriage.
In next pregnancy you should get an early ultrasound at 12-14 weeks to detect any anomaly.
Further management should be done accordingly.
Preconceptional folic acid supplementation should be taken to reduce the chances of chromosomal anomalies.

You can get genetic counselling and prenatal advice in them.
Folic acid 5 mg tablet should be taken from 3 months before planning pregnancy and continued till first three months after conception.
